No. You can book an appointment and pay for it with no email address on your HealAssist account — your mobile number is your identity here, and the booking form marks email optional. Adding an email buys you things, though: an emailed receipt copy, and the download link if you ask for a copy of your data.

What you can do without an email address
You can create a HealAssist account, book a doctor consultation, pay the consultation fee, buy a voucher, receive your notifications, and get your receipt. None of it requires an email address. The email field on the booking form says "optional" because it means it.
What adding an email address actually changes
Two things, and one of them matters more than it looks. First, HealAssist can email you a copy of your receipt. Your receipt exists either way and is in your HealAssist account — the email just gives it a second place to land.
