Scheme paperwork: what to send and how

Attach your scheme card and supporting documents to the coverage check on your scheme’s page — that form takes several files at once, each a PDF, JPG or PNG up to 10 MB. Each scheme page lists what that scheme usually asks for. Your account then lists those files back to you, and you can remove any of them.

Every government scheme runs on paper. The good news is that the list is short and you probably already have most of it; the part worth getting right is how it reaches HealAssist, and what happens to it afterwards.

How do I send my scheme documents?

  1. Check what your scheme asks for

    Each scheme page on HealAssist lists the documents that scheme usually wants — your scheme card, an identity document, sometimes a referral. Treat it as a guide, not a guarantee: the scheme, not HealAssist, decides what is enough.

  2. Attach them to a coverage request

    Start the coverage check from your scheme’s page and attach your documents there — that form takes several files at once, and it is the same upload HealAssist uses for insurance and TPA documents. The shorter form inside your account takes one file at submit, but you can add more afterwards from Recent coverage checks for as long as the request is still open.

  3. Send a clear photo or a PDF

    A PDF, or a JPG or PNG photo from your phone — up to 10 MB each. Every file is checked for viruses. If that check cannot be completed, HealAssist keeps the file but will not open it, and you will need to send it again.

What happens after I send them?

The HealAssist team works your coverage request with the hospital, and coordinates the admission with them. If something is missing or unreadable, they will come back to you — which is another reason the phone number on your account should be one that actually reaches you.
