Multifocal vs Trifocal IOL

Standard (bifocal) multifocal IOLs have two focal points — near (40 cm) and far (6+ m) — which was fine when cataract surgery first emerged. Trifocal IOLs add a third focal point at intermediate distance (60–80 cm), which is exactly where your laptop, car dashboard, and supermarket shelves sit. If you spend several hours at a computer or drive regularly, a trifocal is a significantly better lifestyle fit than a bifocal multifocal.

Bifocal multifocal IOLs (ReSTOR, Tecnis ZMB00, Indian hydrophilic multifocals) cost ₹50,000 – ₹90,000 per eye. Trifocal IOLs (Alcon PanOptix, Zeiss AT LISA tri, Rayner RayOne trifocal) cost ₹90,000 – ₹1,50,000 per eye. The price gap of ₹30,000 – ₹60,000 per eye buys you intermediate vision and, in most modern trifocals, better halo control.

PanOptix (Alcon) is a trifocal IOL — it has three focal points and is technically a diffractive multifocal with a trifocal pattern. It is the most-implanted premium IOL in India today. When a surgeon in India says 'multifocal', it is worth asking whether they mean a true trifocal like PanOptix / AT LISA tri, or an older bifocal multifocal.

Counter-intuitively, modern trifocals often produce fewer severe halos than older bifocal multifocals. This is because newer trifocal optics (PanOptix, AT LISA tri) split light more smoothly across three points. That said, both still produce some halos around lights at night. Extended Depth of Focus (EDOF) lenses like Vivity are the best choice if zero halos is non-negotiable — but you trade off near vision.

Standard health insurance in India covers only monofocal IOL cost (₹12,000 – ₹25,000). Both multifocals and trifocals are treated as patient upgrades — you pay the difference. Some newer OPD-rider policies (Niva Bupa ReAssure, Care Supreme, HDFC Ergo Optima Secure) now reimburse part of the premium IOL cost; always ask for a pre-authorization in writing before surgery.

At 55, you are likely still working at a computer or smartphone daily — a trifocal (PanOptix, AT LISA tri) gives you that intermediate range without reading glasses. At 70+, your daily vision needs are typically distance + reading, and you may adapt more slowly to diffractive optics, so a bifocal multifocal or an EDOF lens is often a gentler and cheaper choice. Discuss your specific daily activities with your surgeon — lens choice is about lifestyle, not just age.
